Becoming Appointed with USHL.

Whether you are an agency or individual agent, becoming appointed with USHL gives you the ability to access our online quoting tools and fully implement new benefit plans with your clients.

We’ve streamlined the appointment process so you can focus on taking care of your clients, utilizing DocuSign so there’s no need to download and file more forms in our Agent Licensing Documents packet.

Complete and Submit Appointment Forms

Complete the Licensing Documents Package and Agent Assignment Form via DocuSign. The package Includes Forms, Agreements, W-9*, Hitech Business Associate Agreement, Copy of Life And Health License, Copy of Certificate For Agent’s E&O Insurance Coverage (or Evidence of Coverage). Individual Agents also complete the Agent Assignment Form, to reassign commissions to another agent or agency.

Finalize your Appointment Status

We’ll connect to confirm all documents are in place and get access to online tools.

Register for Online Quoting

Complete and submit your Online Services Agreement to finalize your access. Upon receipt of required paperwork, a brief online training session will be provided. Please allow 48-72 hours for completion of the registration process.

*Submittal of W-9 is required for both agents and agencies seeking appointment:
Agents not assigning commissions to an agency must submit their W-9 only.
Agents assigning commissions to an agency must submit both their W-9 and also the agency W-9.
